Please return the membership application filled out front and back.  Both parents and the contestant must sign the application.  If you have any questions regarding the application form just give me a call and I will be happy to help you.  I will also need a copy of the contestant's birth certificate and the year end final report card for last year.  All of this must be complete

 before the contestant can rodeo.
